WHAT’S UP WITH CHRONIC LOW BACK PAIN? Spinal Manipulation’s Role
Chronic low back pain like that described by the veteran in this case report is very common and can be quite disabling. Spinal manipulation is recommended for the management of chronic low back in many guidelines backed by such groups as the American Pain Society and the American College of Pain Physicians. (2,3) With chronic low back pain, central sensitization – the effect of repetitive nerve stimulation has on the central nervous system - is thought to play a role in chronicity of pain. Spinal manipulation’s advantage may well come from its influence on this sensitization. (4) That is an advantage chronic back pain sufferers can live with.
SPINAL MANIPULATION BETTER FOR CHRONIC LOW BACK PAIN…REGARDLESS OF AGE
A systematic review and meta-analysis of randomized controlled trials (47 RCTs and 9211 participants) of spinal manipulation treatment for chronic low back pain stated that spinal manipulation effected similar effects to other recommended therapies of chronic low back pain (like exercise and drug therapy), was superior to non-recommended intervention for short-term improvement, and had limited, mild-to-moderate, passing adverse effects. (5) Another such meta-analysis concluded that spinal manipulation appeared to be a good option for pain relief and function improvement for chronic low back pain sufferers. (6) In a meta-analysis of specifically older (ages 65-91 y/o) chronic low back pain sufferers, the same deductions were reported: spinal manipulation should be considered a positive treatment for the older chronic low back pain sufferer to relieve pain and improve function. (7) Most spinal manipulation is delivered by chiropractors according to the National Center for Complementary and Integrative Health.
